Brain Injury Solutions

Brain Injury Solutions passion is to create and sustain an organization that helps brain injury survivors and their families in Southwest Virginia. Our goal is to make a positive, measurable difference in survivors’ abilities to fulfill their service potential and optimize their reintegration into their families and communities. We provide both adult and child and adolescent case management over 12,000 square miles of SWVA as well as community support services in a limited-service area.

Case Management

Case management is the process of meeting individual’s needs through services and resources by the following methods:

  • Coordinating
  • Evaluating
  • Identifying
  • Monitoring
  • Planning

Goals & Responsibilities

A collaborative process is used to identify survivor-centered goals, and marshal (coordinate) resources to meet those goals. In addition, case management includes advocating for and creating community resources needed to assist survivors in maximizing recovery, optimizing employment or service potential, and reintegrating into their families and communities.

Fees

There is no cost for case management services directly provided by a Case Manager. If a person receives a service that is contracted for by their Case Manager, then the Financial Eligibility Form will determine whether the client will be required to contribute towards the cost of a particular service.
